Bears DT Jay Ratliff is being evaluated for a concussion.

Ratliff exited in the third quarter and is likely done for the night. His status will be updated no later than Wednesday.

Charles Tillman suffered a triceps injury in Sunday night's Week 2 game against the 49ers.

He's done for the night. It's a significant injury after Tillman missed the final seven games of 2013 with a torn triceps. The Bears defense is in shambles after losing S Chris Conte in the second quarter. Tillman could be a candidate for injured reserve.

Rod Streater suffered a hip flexor injury in Sunday's Week 2 loss to the Texans.

Streater's departure allowed Andre Holmes (5-45-0 on seven targets) and Denarius Moore (3-29-0 on five targets) to get some extra run. The severity of Streater's injury is unknown right now.

49ers TE Vance McDonald left Sunday night's game with a knee injury, but later returned.

McDonald ended up missing only one quarter. He'll continue to play in two-tight end packages with Vernon Davis.

Lions CB Nevin Lawson dislocated his ankle in Sunday's Week 2 loss at Carolina.

Lawson needed immediate surgery, which means the fourth-round rookie is likely done for the year. The Lions' already weak secondary is now even thinner after also losing Bill Bentley (torn ACL) in Week 1.

Bears FS Chris Conte was forced from Sunday night's game in the second quarter with a shoulder injury.

He's done for the night. Conte had two tackles and an interception before exiting. Danny McCray has replaced him in base sets.

According to CBS Sports' Jason La Canfora, "early indications" are Jamaal Charles' ankle sprain is "mild."

The same goes for SS Eric Berry, who like Charles exited Sunday's loss to Denver and did not return. Per La Canfora, the team is "hopeful neither will miss significant time." Charles is believed to be scheduled for a Monday morning MRI. It does sound like Charles has a shot to play in Week 3 at Miami, but Knile Davis needs to be owned everywhere. The fact that Charles never reentered after the first-quarter injury suggests he was in a lot of pain.

The Union-Tribune San Diego confirms the "initial prognosis" for Ryan Mathews' right knee injury is a sprained MCL.

It remains "unclear how much time" Mathews will miss. Mathews is expected to be "further evaluated," but a sprained MCL would be good news at this point, as opposed to an ACL tear. Mathews could be back within a month. The Chargers' bye isn't until Week 10, so Mathews won't have that date to work around. Look for Donald Brown to take over as San Diego's primary back while Danny Woodhead continues to handle most passing-down and change-of-pace work.
