Rockies prospect Trevor Story turned in another strong outing Sunday for Double-A New Britain, finishing the day 4-for-4 with a home run and two doubles.

That's become the norm for Story, who is now hitting .395 in 81 at-bats and .444 over his last ten games. With five homers and 21 RBI, the 21-year-old is quickly outgrowing Double-A. In addition to being a prodigious power hitter, Story is also a threat to steal bases. After swiping two bags Sunday, he now has six on the season.

Braves prospect Jose Peraza clubbed four hits Sunday in a win for Triple-A Gwinnett.

Peraza added three RBI, raising his season total to seven. The 21-year-old rarely strikes out (ten punchouts this year) and hits for a high average (.301 in 93 at-bats). The problem is, he doesn't work deep into counts and has no power to speak of (no home runs). When Peraza does get on base, he's always a threat to steal. So far, he's recorded seven thefts on eight attempts. The second baseman could make his big league debut later this year.

Indians manager Terry Francona said Nick Swisher (knees) will be activated from the disabled list Tuesday.

Tyler Holt will be optioned to Triple-A Columbus to open a spot on the Indians' 25-man roster. Swisher missed all of April while rehabbing from dual knee surgeries. He batted just .208/.278/.331 with eight home runs and 42 RBI in 97 games last season for the Indians and the 34-year-old seems doubtful to carry significant value this summer in standard fantasy leagues.

Astros prospect Domingo Santana blasted a pair of homers Sunday in a win for Triple-A Fresno.

Santana used Sunday's bounce-back effort to snap a 2-for-14 drought. The 22-year-old is batting a steady .286, though it's come with 29 strikeouts in 70 at-bats. This could be the start of something big as Santana's production is known to come in spurts. He already has two multi-home run games this season. Santana is MLB.com's 70th-ranked prospect.

Rule 5 Draft pick Logan Verrett cleared waivers and has been returned to the Mets.

Verrett was selected by the Orioles in December's Rule 5 Draft and then claimed off waivers by the Rangers in early April. Now he's back with the organization that made him a third-round pick in the 2011 MLB Amateur Draft. Verrett, 24, allowed seven runs -- six earned -- in nine innings of relief this season with Texas. He'll join the Mets' Triple-A affiliate in Las Vegas.

Ben Zobrist (knee) has begun playing catch.

Zobrist is already itching to get back, though he's not expected to be fully ready until sometime in early-to-mid June. He rode a stationary bike for 20 minutes the day after his April 28 arthroscopic knee surgery.

Jarrod Parker (elbow) allowed three earned runs over 4 1/3 innings Sunday in a rehab start at Triple-A Nashville.

Parker walked one and struck out four. It was the third rehab outing for the 26-year-old right-hander, who underwent the second Tommy John reconstructive elbow surgery of his career last March. He should become an option for the A's starting rotation later this month.

Jean Segura's CT scan turned up negative.

Segura got drilled in the helmet by a Pedro Strop fastball on Sunday afternoon and was shaken up after the game, but it seems he avoided a serious injury and might not even need a disabled list stint. There should be more on his status Monday evening. The 25-year-old shortstop is batting .289/.324/.381 with one home run and five stolen bases in 25 games this season.
