Marouane Chamakh continued his rebirth under Tony Pulis in Week 16 despite his team losing on their visit to Stamford Bridge.

The former Arsenal forward, along with the entire Palace team, has been much improved since it became clear that Tony Pulis would become their new manager. The goal was Chamakh's third in his last three matches and came from a Joel Ward cross. Palace fought valiantly against the more talented Blues but couldn't find a second goal to take a point home with them across London.

Jack van Gelder, the host of Dutch sports television channel Ziggo Sport, claims Manchester United midfielder Paul Pogba has been carrying a secret injury for almost two months.

Following the 2-0 win over Crystal Palace, Van Gelder told Ziggo that Pogba’s return "went well" before going on to suggest the 24-year-old had been receiving treatment from Dutch physiotherapist – Leo Echteld – for the past eight weeks to help cure a troublesome hamstring injury. Pogba played a little under 45 minutes on Sunday having missed the two previous matches following the death of his father, as Jose Mourinho carefully manages his £89m star ahead of Wednesday night’s Europa League final against Ajax. United made seven changes for the visit of Sam Allardyce’s men on the final day and it was one of those newcomers, Josh Harrop, on his senior debut, who set the home side on their way to victory with a stunning opening goal.

Arsène Wenger has been left sweating on the fitness of Shkodran Mustafi with Laurent Koscielny and Gabriel Paulista likely to miss next weekends’ FA Cup final.

The German international was a notable absentee from the Arsenal line-up against Everton and it wasn’t until after the game that the reasons behind his omission was known. Mustafi is being treated for concussion after the centre-back complained of a headache and dizziness following the 2-0 win over Sunderland. But while it was initially thought that he would be okay to face Chelsea at Wembley on Saturday, the problem could be a little more serious, and his availability cannot be guaranteed at this stage. With Koscielny suspended and Gabriel [knee] ruled out after he was stretchered off yesterday, Wenger could be forced into playing Monreal alongside Rob Holding and Per Mertesacker with Kieran Gibbs returning to the side having recovered from a thigh injury.

Mousa Dembele will miss Tottenham’s end of season trip to Hong Kong as he is scheduled to undergo foot surgery this week.

The 29-year-old has struggled during the latter part of the season due to a persistent foot problem and the club have taken the decision for the midfielder to go under the knife in an attempt to cure the long-standing injury. It’s hoped that Dembele will be ready to return for the start of pre-season at the beginning of July. Kyle Walker will be included in the squad, even though he is being heavily linked with a summer move away, and in spite of missing the 7-1 win at Hull because of an ankle issue. However, Danny Rose [knee] and Erik Lamela [hip] will not be a part of the travelling group after the pair had more surgery earlier this month. In addition, Harry Winks [ankle] remains on the sidelines.

The David Moyes era at Sunderland has come to an abrupt end after the manager handed in his resignation on Monday.

The Black Cats were the first team relegated from the Premier League this season and finished last on just 24 points from six wins, six draws and 26 losses. After just one year, Moyes couldn't reverse the one-and-done trend after stints at Manchester United and Real Sociedad. Said chairman Ellis Short in a statement: "Having worked tirelessly throughout the campaign to avoid relegation from the Premier League, David has chosen to leave the club without compensation, which is testament to his character. In the days ahead we will take some time for reflection, and then focus on recruitment and pre-season as we prepare for our Championship campaign. We wish David well in the future."

Manchester City defender Gael Clichy confirmed that he will not return to the club next season.

In an interview with "Sky Sports" at a fan Q&A event, Clichy said that he has played his last game at City when asked whether he would play alongside Joe Hart next year: "It's hard. I'm not sure whether or not Joe will be here next season, and I won't be here next season." Clichy's contract is up this summer and looks to be the second City defender to confirm his exit with Pablo Zabaleta receiving a sendoff last week at the Etihad. The Frenchman joined from Arsenal in 2011 and won two Premier League titles with the side.

Leicester City has announced the departure of defender Marcin Wasilewski after four years at the club.

The 36-year-old was a key part of the club's title-winning team last season as the first defender off the bench, which lead to a contract extension, but the defender made only one appearance in the Premier League this season. He will now leave the club on a free transfer, four seasons since his move from Anderlecht. Said manager Craig Shakespeare: "Marcin’s been an absolute pleasure to work with over the last four years. The influence he has had and the contribution he has made goes beyond what people see on the pitch. He’s one of the best professionals I’ve ever worked with."

Tottenham finished the season on a very positive note as Harry Kane scored a hat trick for the second time in three days and Spurs got goal-happy, ripping apart Hull City 1-7.

Kane scored seven goals in less than 72 hours. Incredible. He finishes the season with 29 goals in league play alone and grabs his second straight Golden Boot. It looks pretty safe to say, this is no one-season wonder, as the away supporters chanted during the dazzling display. Too many goals to provide detail, but Dele Alli, Victor Wanyama, Ben Davies and Toby Alderweireld also got on the score sheet. Christian Eriksen provided a pair of assists as well as Kieran Trippier, who looks poised to try and claim the right back role next season with Kyle Walker heavily linked to an exit from the club in the summer. According to Opta, Spurs +60 goal difference is the highest in league history for a non title-winning side. The future certainly looks bright enough though.

Burnley's strong season came to a poor end as they were defeat 2-1 by West Ham at Turf Moor.

The Clarets took an early lead when Sam Vokes scored his 10th league goal of season, but a strike by Sofiane Feghouli four minutes later and a late header by Andre Ayew proved too much for the home side to overcome. Burnley were unlucky to concede what ended up being the winning goal as Tom Heaton did well to stop Emilison Fernandez but the ball dropped kindly to Ayew, who headed into a wide open net from inside the six-yard box. All things considered, though the results sees them finish in 16th, it was fine season for Sean Dyche's side. The club ends the term having picked up 33 of a possible 40 points at home, while securing top-flight football in consecutive seasons for the first time in the Premier League era.

Liverpool are headed back to the Champions League after securing a top-four finish by defeating Middlesbrough 3-0 at Anfield.

All eyes were on Jurgen Klopp's side heading into the final day of the season, with the Reds in need of a win to lock up a place in Europe. However, despite a slow opening 45 minutes, the club would not be denied. Georginio Wijnaldum delivered the vital opener with a rocket from inside the area just before the break while Philippe Coutinho doubled their advantage just six minutes after the restart, as the Brazilian curled a free-kick over the wall and down into the bottom right corner. With the match seemingly in hand, Adam Lallana added the third when he cooly slide a left-footed strike past Boro goalkeeper Brad Guzan. The result sees Liverpool return to the Champions League for just the second in the last eight years.

West Ham United's bumpy campaign came to an end with a positive result as Andre Ayew headed the Hammers to a 2-1 victory over Burnley.

Slaven Bilic's side endured a rocky 2016/17, but an early strike by Sofiane Feghouli and late goal by Ayew were enough to salvage a come from behind win at Turf Moor. Burnley was first strike when Sam Vokes finished from close range to put them ahead inside the opening 25 minutes. However, Feghouli wasted no time drawing the Hammers level as he tucked a well-struck strike from the top of area underneath Tom Heaton. While the Clarets went on to control the majority of chances, it was the visitors who made their best opportunity of the second-half count. Edimilson Fernandes was the creator as he blasted a shot on net, which Heaton palmed off the bar and fell kindly for Ayew to head home. The result sees West Ham finish in 11th, on 45 points.

Southampton's struggles continued as they dropped a 1-0 decision to Stoke City on the season's final day.

Manager Claude Puel has come under increasing fire as his side have limped down the stretch of the 2016-17 Premier League season. The Saints were never in any danger of relegation and finished in 8th position but Sunday represented their fifth consecutive home match without a goal. Given the amount of money spent on reinforcements like Sophiane Boufal and Manolo Gabbiadini, Saints supporters would have expected more and there have been increasingly loud cries for the French manager to be relieved of his duties. It will be an interesting summer at St. Mary's.
