Lance Berkman (hip) admitted Wednesday that he's "probably still not quite ready" to return to the lineup.

In addition to his hip ailment, Berkman also has chronic soreness in his right knee, which he has had operated on four times. He is taking batting practice but has yet to do any real running. "He's progressing, but it's slow," Rangers manager Ron Washington said. "We just have to let it go through the process. He'll let us know when he's ready."

Indians placed LHP Andrew Miller on the 10-day disabled list with right knee patella tendinitis.

Miller aggravated his knee injury during an appearance against the Red Sox on Monday night. He missed about two weeks last time and could require a longer absence this time around. More should be known in the coming days.

David Wright (shoulder) will begin a minor league rehab assignment Tuesday with High-A St. Lucie.

He's set to serve as the designated hitter in his first professional game of the season. Wright has been out all year due to a shoulder impingement while trying to manage his back condition. It's no sure thing he'll be able to join the Mets down the stretch, but it would at least provide some smiles for Mets fans in what has been a lost year.

Royals activated C Salvador Perez from the 10-day disabled list.

Perez ended up missing a little over two weeks with an intercostal strain. His return should provide a boost to the Royals as they attempt to climb in the AL Wild Card race.

Zack Cozart (shin) has returned to the Reds' lineup Tuesday against the Cubs.

Cozart hasn't played since he was hit in the shin area by a pitch during Friday's game. He'll bat second against right-hander John Lackey in his return to action.

White Sox recalled RHP Lucas Giolito from Triple-A Charlotte.

Giolito was originally scheduled to start in Monday's doubleheader, but he was pushed back to take Reynaldo Lopez's spot in the rotation Tuesday against the Twins. Acquired from the Nationals as part of the Adam Eaton deal, the 23-year-old posted a 4.48 ERA and 134/59 K/BB ratio in 128 2/3 innings over 24 starts in Triple-A this year. His stock has faded somewhat, so he's more of a wait-and-see option in standard mixed fantasy leagues at this time.

Vince Velasquez will undergo surgery to repair a vascular issue with his right middle finger.

Phillies manager Pete Mackanin told Meghan Montemurro of the News Journal that the issue has been clogging blood flow to his middle finger. This would explain the numbness he has been experiencing. Velasquez is officially done for the year and his role for 2018 could be up in the air.

According to Jon Heyman of FanRag Sports, the Phillies are set to sign free agent right-hander Henderson Alvarez.

It's expected to be a minor league deal. He'll make a prorated salary of $750,000 if he's called up by the Phillies. Alvarez hasn't appeared in a major league game since May of 2015 due to shoulder issues, but Heyman writes that he's been hitting 98 mph while pitching for the independent Long Island Ducks, though he had more walks (14) than strikeouts (13) in 32 innings. We're not expecting much from a fantasy perspective.

Yankees activated RHP Masahiro Tanaka from the 10-day disabled list.

Tanaka went down a week and a half ago with shoulder inflammation, but he'll make his return Tuesday against the Tigers. The hope is that the extra rest will get him back on track for the stretch run. Tanaka uncharacteristically walked five batters over four innings in his most recent start back on August 9.

Trea Turner (wrist) went 0-for-3 with a strikeout Monday in his first minor league rehab game with Triple-A Syracuse.

Turner also played five innings at shortstop in his first game action since late June. The dynamic shortstop will likely need a few more rehab games as he works his way back from a fractured right wrist. He could be a difference-maker down the stretch in fantasy leagues.

Yasiel Puig cracked a go-ahead solo homer in the top of the 12th inning Monday as the Dodgers rallied to beat the Pirates 6-5.

Another clutch moment for Puig, who also caught the final out on Max Moroff's liner to right field in the bottom of the 12th. Puig drew a walk in his first plate appearance of the night, doubled in his fourth plate appearance of the night, and went deep off Pirates reliever Dovydas Neverauskas in his sixth plate appearance of the 12-inning affair. The rejuvenated 26-year-old outfielder owns an .803 OPS with 22 home runs and 58 RBI in 117 games this season for a Dodgers team that sits 53 games above .500.

Byron Buxton stayed hot Monday against the White Sox by going 2-for-4 with a solo homer and two runs scored.

In doing so, Buxton extended his hitting streak to a career-best 12 games. He’s hit four homers (including one inside-the-parker) with 12 RBI and 12 runs scored during the streak. It’s time to forgive and forget in mixed fantasy leagues. Buxton is well worth a shot for the stretch run.
