Matt Reynolds (elbow) threw Monday for the first time since undergoing Tommy John surgery last September.

Reynolds initially attempted to rehab the ulnar collateral ligament tear in his left elbow, but eventually had to go under the knife. The 29-year-old is unlikely to contribute until late 2014 at the earliest.

A.J. Pollock (hand) began a rehab assignment with the Diamondbacks' Arizona League team on Thursday.

He was retired in his lone at-bat, but that hardly matters. Pollock has been sidelined for the last two months after breaking his right hand and is expected to need a couple weeks' worth of rehab at-bats before being activated. He was hitting .316/.366/.554 with six homers, 15 RBI and eight steals before getting hurt, so feel free to stash him now if he's been dropped in your league.

Angel Pagan (back) doubled twice in three at-bats in his first rehab game with the Giants' Arizona League club on Thursday.

Pagan's back ailment has progressed well over the past week and he didn't show any rustiness in his first game action in seven weeks. It's not clear how many rehab games the outfielder will need, but he's close to rejoining the Giants' lineup.

Jose de Jesus Ortiz of the Houston Chronicle reports that the Astros will call up pitching prospect Michael Foltynewicz from Triple-A Oklahoma City on Friday.

Ortiz says the Astros plan to use Foltynewicz as a reliever. Eighteen of the 22-year-old's 21 appearances at Oklahoma City this season have been as a starter, but many scouts ultimately see Foltynewicz as a reliever over the long haul. Ranked prior to the season by Baseball America as the team's No. 4 prospect, Foltynewicz holds a 5.08 ERA but has struck out a batter per inning at Triple-A. His fastball can reach triple digits.

Eric Hosmer (hand) could be sidelined for up to six weeks.

The initial timetable floated was four weeks, but apparently Hosmer could be on the shelf even longer than that. The first baseman is dealing with a stress fracture in his right hand, an ailment that he originally suffered over a week ago but played with after initial X-rays didn't find anything. The Royals will go with a combination of Billy Butler and Raul Ibanez at first base while Hosmer is sidelined.

Tyler Skaggs (forearm) is expected to be placed on the disabled list.

Skaggs' elbow ligament is fine and the Angels are confident his forearm tightness is a minor issues, but they don't want to take any chances. He'll undergo an MRI Friday before the Halos officially make a decision. "The doctor said the UCL [ulnar collateral ligament] is fine," Skaggs said. "At the moment, it’s not looking bad. I have full range of motion, and the tests went well. We’ll see tomorrow, though. It’s not feeling too bad right now."

Jason Heyward (back) is hoping to return to the Braves' lineup on Saturday.

Heyward has sat out three straight games with a sore back, but he started to feel some relief on Thursday. He'll hope to play in the final two games of the weekend series in San Diego.

Clayton Kershaw dazzled again in Thursday's victory over the Braves, racking up nine strikeouts and allowing only one run in a 2-1 complete-game victory.

Kershaw walked none and allowed nine hits in his 10th straight victory -- his first regular season win over the Braves, the only NL team he'd never vanquished in the regular season -- and the Dodgers' sixth consecutive win. He's allowed only nine runs in 86 innings over his last 11 starts. Having improved to 13-2 with a 1.71 ERA, Kershaw hosts the crosstown Angels on Tuesday.

Albert Pujols broke a 0-0 tie in the 13th inning with an RBI single, finishing 2-for-6 overall in Thursday's win over the Orioles.

After the Angels had Kole Calhoun and Mike Trout reach base with a walk and a single, respectively, Pujols ended 12 innings of offensive futility with a single up the middle to score Calhoun. Stunningly, it was Pujols' first RBI since July 23. The 34-year-old has a fine .274/.324/.471 slash line with 20 home runs and 67 RBI on the season.

Ben Revere finished with four hits and a stolen base in six at-bats as the Phillies handily defeated the Nationals on Thursday.

Thursday's game didn't start off so well, as Cliff Lee quickly exited with an injury, but Revere did his best out of the lead-off spot to ensure a victory. It's Revere's third four-hit game of the season, and the stolen base brings him up to 30 on the season in 34 opportunities. Acknowledging his total lack of power, Revere has done his job at the plate thus far, slashing .306/.326/.366.

Phillies placed LHP Cliff Lee on the 15-day disabled list with a left flexor pronator strain.

It's an aggravation of the injury that sidelined him for two months earlier this season. Lee might require surgery this time around, though that won't be known until he's examined Saturday. Either way, he's likely done for the season. The veteran lefty is owed approximately $45.8 million through the end of next year.

Nolan Reimold hit two solo home runs on Thursday, including a go-ahead shot in the ninth inning to help the Blue Jays edge out the Astros.

The two homers were his only successes in four at-bats, but they were big successes. His first occurred in the fifth off of starter Jake Buchanan, and his second came against reliever Chad Qualls to break a 5-5 tie in the ninth. Reimold recently returned from a calf injury and hasn't played much since joining the Jays earlier this season. In 19 plate appearances with the Jays, he has the pair of homers along with three doubles and five RBI.
