With Jonathan Papelbon needing the day off, Chad Qualls got the call in the ninth and blew a save Tuesday versus the Astros.
Qualls allowed two runs and four hits in two-thirds of an inning to cost Cliff Lee the win. It's already the fourth time he's been charged with a blown save this year, though this was the first time he was actually put in position to close out a game.

Chad Qualls tossed a scoreless inning Saturday in his return to the mound.
He picked up a strikeout while pitching around a walk and a hit in the frame. Qualls had been limited recently by a left knee injury, but it doesn't appear to be an issue anymore.

Updating an earlier item, Chad Qualls (knee) has not been placed on the disabled list.
Qualls is nursing a knee ailment that was expected to land him on the DL, but the Phillies evidently think he'll be ready to pitch before long. The right-hander holds a 3.48 ERA and 1.65 WHIP over 12 appearances this season.

Chad Qualls is expected to be placed on the disabled list with a left knee injury.
It's not clear when Qualls was injured, but Phillies manager Charlie Manuel said after Wednesday's game that Qualls was unavailable even though he had only thrown three pitches in the previous five days, and then the righty was later seen wearing a brace on his knee. He's expected to be one of three moves made by the Phils, as Laynce Nix (calf) is also likely headed to the DL, and Joe Savery has already been optioned.

Phillies general manager Ruben Amaro on Sunday defended the club's use of a cortisone shot to treat Ryan Howard's foot last September.
Howard tore his ACL less than three weeks after receiving the shot. "I wanted to clear up some of the insinuations regarding a cortisone shot," Amaro said. "The cortisone shot was treated for some (other) issue he had. It was not part of the Achilles injury. We didn't feel it was an issue. That was resolved by the time he had his injury. One thing had nothing to do with the other." A doctor quoted by the Philadelphia Inquirer disagreed. "When it comes to the risk of an Achilles tendon tear, I'm concerned about cortisone," he said. Howard is tentatively expected to return to the Phillies sometime next month.

Jim Thome (back) played in an extended spring training intrasqaud game Thursday.
Thome has been sidelined for nearly a month now with a lower back strain. It's not clear when he'll be ready for an official minor league rehab assignment.

Chase Utley (knee) felt "pretty good" after fielding grounders Wednesday.
In fact, Utley felt good enough to run Thursday and plans on fielding grounders for a second straight day. "I'm trying to get to a point where when we start going for it, I feel pretty comfortable," said Utley. "I've realized over the course of this time that the more I push it and feel uncomfortable, the worse it can get. We try to make small progressions. Taking ground balls yesterday was a good sign. I felt pretty good today. I plan on doing the same." Utley is clearly making progress, but there are no immediate plans to send him on a minor league rehab assignment.

Phillies transferred INF Michael Martinez from the 15-day disabled list to the 60-day disabled list.
The move clears a spot on the 40-man roster for Mike Fontenot. Martinez has been on the disabled list all season after breaking a bone in his right foot during spring training.

According to the Philadelphia Inquirer, the recovery timetable for Laynce Nix (calf) is a matter of weeks, not days.
Nix was diagnosed with a Grade 2 left calf strain on May 10, and was just recently cleared to begin exercises. "It's going to be longer than the 15 days," Nix said. "It's a pretty good tear. It's unfortunate." Nix is out indefinitely and remains without a timetable.

Vance Worley (elbow) said he was pain-free playing catch Thursday.
Worley visited Dr. James Andrews earlier this week, who confirmed the Phillies' diagnosis of a bone chip and inflammation in his elbow. The 24-year-old right-hander may require offseason surgery, but he'll try to pitch through the condition for now. He's slated to play catch again Friday and will slowly ramp up his throwing program from there.

Michael Stutes (shoulder) will visit a Phillies team doctor Monday and could be cleared to resume throwing at that time.
Stutes has been sidelined since April 22 with right shoulder soreness. If he does receive clearance, he'll need a few bullpen sessions before going out on a rehab assignment.

Phillies placed RHP David Herndon on the 15-day disabled list with a flexor pronator strain in his right elbow.
He'll be shut down for three weeks but is not a candidate for surgery right now. Herndon gave up two runs in two innings during his most recent appearance Sunday and owns a 4.70 ERA and 8/1 K/BB ratio over 7 2/3 innings this season. The Phillies will make a corresponding roster move Tuesday.
