John Terry lifted his fifth Premier League trophy on his final day in Chelsea blue following a 5-1 win against Sunderland.

It was a precarious start for the newly-crowned champions after conceding a goal in the third minute to temporarily put a damper on the title celebrations. But Willian got Chelsea on the board in the eighth minute and it was off the the races from there with four goals after the 60th minute. Eden Hazard scored his 16th in the 61st, Pedro stole a goal in the 77th, and Michy Batshuayi scored two as a second-half substitute in stoppage time to conclude the scoring. But it was Terry's day as the long-time captain was taken off in the 26th minute to a roaring ovation and a farewell that ended in more Premier League silverware.

West Ham United have turned their attention to this summer's transfer window, and are keen to sign Chelsea's title winning hero Michy Bastshuayi on loan.

Batshuayi, who scored the goal that sealed the title for Chelsea against West Brom, has found play time hard to come by at Stamford Bridge, and West Ham are ready to offer him a chance to shine. According to reports, Hammers are hoping to take Batshuayi on loan next season having missed out on signing the Belgium international from Marseille last summer. Batshuayi is however not the only striker on the London Stadium side's wishlist, with the club also weighing up moves for Manchester City's Kelechi Iheanacho and Sunderland's Jermain Defoe.

Chelsea defender John Terry did not rule out retirement as a potential option this summer after Monday's win.

The Chelsea captain scored in a 4-3 win against Watford and will play his final home match at Stamford Bridge on Sunday against Sunderland. With Bournemouth and Swansea City managers already confirming their interest, Terry said he has not decided on his future yet: "I know I said I wanted to play regular football, but I have not ruled out Sunday being my last game and retiring. It depends if the right offer comes along then I will sit and consider it with my family, whether it's here or abroad, but I haven't made any decisions yet and I am just evaluating my options."

Bournemouth manager Eddie Howe is reportedly keen to bring former loanee Nathan Aké and Asmir Begovic to the Vitality Stadium.

The Cherries' summer objective is to bring players to the club that will take them to the next level in the PL after consolidating their status in the division this season. Aké, 22, had a very successful loan spell with the Cherries until he was recalled by his parent club Chelsea back in January. Since, he has been a backup option for Conte, as has goalkeeper Asmir Begovic. Bournemouth approached Chelsea for the pair back in January but both their attempts failed. In the case of Begovic, Conte would not let him go until a suitable understudy to Courtois was secured. For Aké, Chelsea are reluctant to sell given his obvious potential to be a first team player for them in the future. However, Bournemouth may start by offering as high as £20 million for his services. That would be difficult for them to refuse.

Chelsea captain John Terry has become the 4th player to score in 17 consecutive Premier League seasons after netting the opener in Monday's 4-3 win over Watford.

Set to leave Chelsea this summer after 22 years at Stamford Bridge, Terry made his first league start since September on Monday night and needed less than a half-and-hour to make his presence felt. The 36-year-old was first to react following a corner in the 22nd minute as the ball fell to him to bundle home inside the six-yard box. Terry, who's misplayed header back to Asmir Begovic allowed Etienne Capoue to bring Watford level just two minutes later, now joins Frank Lampard, Ryan Giggs and Paul Scholes who have also scored in 17 successive seasons.

Substitutions once again lead to a late winning goal for Chelsea as Cesc Fabregas scored two minutes from time to lift the Premier League champions to a 4-3 victory over Watford on Monday.

The goals were flowing at Stamford Bridge as Chelsea celebrated winning the Premier League by pulling off an entertaining victory. Antonio Conte made nine changes to his side that clinched the title at West Brom on Friday, as John Terry made a rare start and scored the opener in the 22nd minute. While Terry's misplayed header then allowed Etienne Capoue to equalize shortly thereafter, Cesar Azpilicueta hammered in a strike from outside the box to restore the lead 12 minutes later. Michy Batshuayi was next to strike, to take back the two-goal lead in the 49th minute, but Dayrl Janmaat answered with a stellar solo goal two minutes later. Chelsea's title party once again looked in doubt when Stefano Okaka scored from close in the 74th, but super sub Fabrgas fired into the bottom corner in the 88th to reignite the home fans joy, and give the Blues their 29th win of the season.

Chelsea clinched its second Premier League title in the last three seasons as substitute Michy Batshuayi scored late to secure a 1-0 victory at West Brom on Friday.

In a season highlighted by Antonio Conte's masterclass tactics, it was again a decide by the Italian boss that produced their title-clinching victory at the Hawthorns on Friday night. Despite dominating the first half, Chelsea failed to registered a shot on target until Victor Moses was denied by West Brom goalkeeper Ben Foster just after the restart. However, following a pair of chances by the home side, Conte then made a pivotal double substitution, bring on Willian and Batshuayi in exchange for Eden Hazard and Pedro. And the move paid off just 12 minutes. Cesar Azpilicueta was the provider as he collected a mishit shot by Gary Cahill and sent a low cross into the box to Batshuayi, who slide to poke home from close range and give the Blues the title with two games left to go. The result sees Chelsea crowned champions for the 5th in the Premier League era, while Conte becomes the 4th manager to win the title in his first year in charge.

Michy Batshuayi slotted home from Cesar Azpilicueta for the only goal of the match as Chelsea beat West Bromwich Albion 1-0 to claim the 2016-17 Premier League title.

It has been a frustrating season for Batshuayi but one that he can ultimately remember for this one moment. Brought in over the summer for a significant transfer fee, he spent most of the season watching as an expensive deputy for Diego Costa. Again performing that role after coming on in the 76th minute it only took Batshuayi 6 minutes to net the winner. With Costa's future at Stamford Bridge in doubt, perhaps this will be the goal that helps Batshuayi come good on his transfer fee. Regardless, Chelsea are the 2016-17 Champions of England with two matches remaining.

Celtic manager Brendan Rodgers has praised the "fantastic talent" of Chelsea forward Dominic Solanke after being linked with a move for 19-year-old.

Reports claim Solanke has refused a new contract offer at Stamford Bridge and that Rodgers is interested in bringing him to Parkhead in the summer. Under cross-border rules, the Scottish Premiership champions would only need to pay a fraction of the fee English clubs would have to for the England Under-21 international. While Rodgers did not say he had a definite interest in Solanke when asked about a potential move, the former Liverpool boss admitted the youngster is the type of player that would be of interest to Celtic, who signed Moussa Dembele from Fulham under similar circumstances last summer. "I took Dominic into Chelsea when he was eight years of age. I am delighted for him and the career he has had up until now as a young player, he is a fantastic talent," Rodgers said. "It is unfortunate that he is maybe not going to renew his contract at Chelsea because it is a great club, but he maybe deems that his future lies elsewhere."

Chelsea star forward Diego Costa played down rumors of a move to the Chinese Super League, but didn't rule out a summer exit completely.

Multiple reports linked the Brazilian-born, Spanish international with a move this summer due to his celebration after scoring his 20th goal of the season in a 3-0 win against Middlesbrough on Monday. While joining Jimmy Floyd Hasselbaink and Didier Drogba as the only Chelsea players with multiple 20-goal seasons, Costa appeared to wave goodbye to Chelsea supporters at Stamford Bridge. He was reportedly on the verge of joining TIanjin Quanjian in the winter window, but reports say talks have yet to begin over a move. "I am the owner of my future and it does not mean that I am going to China," Costa said after the match.

Cesc Fabregas has become the first player ever to record at least 10 assists in six different Premier Leagues season.

Fabregas assisted twice in the 3-0 victory over Middlesbrough on Monday, as the Spaniard achieved the milestone accolade to help Chelsea move within three points of the Premier League title. Fabregas was nothing short on excellent, unlocking the visitor's defense throughout from a deep-lying midfield role on just his 11th start of this season. The 30-year-old set up Diego Costa's opener with a well-weight ball over the top in the first half before teeing up Nemanja Matic for the third in the 64th minute, orchestrating the attack with N'Golo Kante out injured. Fabregas has now recorded four assists in his last three appearances, and possibly earned himself a place at Stamford Bridge for next season.

Chelsea are three points away from claiming the Premier League title as they relegated Middlesbrough with a 3-0 victory at Stamford Bridge on Monday night.

Antonio Conte's side played with the swagger of champions with goals from Diego Costa, Marcos Alonso and Nemanja Matic, and can now clinch the title if they win at West Brom on Friday. Chelsea got off to a roaring start and found themselves ahead in the 23rd minute when Costa scored his 20th goal of the season, as the Spaniard collected a Cesc Fabregas ball over the top and tucked his finish between the legs of a charging Brad Guzan. The Blues would doubled their advantage 11 minutes later thanks to Alonso, who darted onto a well-weight pass by Azpilicueta and slide a left-footed strike off the inside of Guzan's leg and into the net. After the restart the home side's offensive assault continued, with them adding the final goal just after the hour mark through Matic, who controlled with his chest before nutmegging Guzan with a rocket from inside the penalty area. The result puts Chelsea seven points clear of Spurs, and in firm position to win a second title in three seasons.

The Telegraph are reporting today that Bournemouth are "confident" of signing Premier League legends John Terry and Jermain Defoe on free transfers.

Manager Eddie Howe is looking to add experience to his side which have proven themselves well capable of thriving at the highest level. Both men are approaching the end of their careers and coming to the end of their contracts at their respective clubs. Upon signing, they will probably be among the club's highest earners on over £100,000 a week. Defoe, 34, played for the Cherries alongside manager Eddie Howe back in 2000/01 in a loan deal to what was then a third-tier club. He is likely to seek a contract which will keep him on the south coast for more than one season. West Ham are also rumoured to be tracking the English international.

Having scored 26 goals in all competitions while on loan at Championship side Bristol City, Chelsea are reportedly ready to offer striker Tammy Abraham a new contract.

Abraham has two years left on his contract at Stamford Bridge and, although he might not factor into Antonio Conte's immediate plans, Chelsea are keen to secure his long-term future. The Blues have high hopes for the 19-year-old after his superb season at Bristol City, scoring 23 goals and recording three assists in 40 league appearances for the Robins. A number of clubs have already expressed interest about the England under-21 international, but Chelsea have reportedly ruled out selling him as they have yet to decide what to do with Abraham next season. Should the highly-rated youngster not be in Conte's plans next term, then a loan to a Premier League club would likely be in the cards.

Eden Hazard has not done himself any favors with Chelsea fans hoping the star will avoid the pull of a big summer move.

The Chelsea winger has been linked with a move to Real Madrid and Paris Saint-Germain after a bounce-back season. When asked about signing a new contract, Hazard gave the standard line, saying he will wait until the end of the season to assess his situation: "There is no sign of a new contract for the moment. I am focused on the last month, and we will talk later. It's not in my mind now. I have two or three years left. I just want to finish the season well and we will see afterwards."
